If you are providing information about more than one birth, please enter the information separately. If you are using your phone to photograph your bill, please use the highest resolution available.
For those of you who do not have a copy of your hospital bill, some insurance companies do itemize statements. If your insurance company does not itemize statements, or you did not have insurance when you gave birth, call the billing department of the hospital you gave birth in. My insurance company would not provide an itemized bill for me, but the hospital did, even though a few years have passed since I had my baby.
By uploading your medical bill you are allowing me to use it in an art installation. If you did not black out your personal information enough and any personal information can be read, I will completely black it out myself so all of your information remains private.
